The ‘One Team’ model of working is based on a collaborative approach to support vulnerable communities through tackling the social and economic drivers of deprivation. This is achieved through; targeted work with those individuals and households who create a disproportionate service demand and by encouraging local residents to take an active role in community development and regeneration.
The success of the ‘One Team’ approach relies on a high level of partnership working between all parties; this close working relationship will involve the sharing of information between professionals and local agencies to allow for effective identification, assessment and service provision and to facilitate a coordinated response.
The approach is now embedded across Somerset and supported by the Think Family Strategy and the Safer Somerset Partnership
